Stellar Health partners with CenterLight Healthcare to enhance senior care

Stellar Health, a US healthcare technology company specialising in value-based care, has entered a strategic partnership with CenterLight Healthcare, the largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in New York.

The partnership focuses on improving health outcomes and provider engagement for CenterLight’s senior members across the region.

CenterLight Healthcare delivers comprehensive services through 11 PACE centres, one owned alternative care setting, and a network of contracted healthcare providers. The collaboration will leverage Stellar Health’s point-of-care platform to help providers optimise care delivery, streamline quality incentive programs, and integrate actionable care tasks into daily workflows. Monthly financial incentives, potentially shared across care teams, aim to encourage provider engagement and improve population-level patient outcomes.

The partnership will expand upon Stellar Health’s existing presence in New York, where thousands of healthcare providers already use the platform. Stellar Health’s platform provides in-workflow prompts to efficiently close care and quality gaps. With over 20,000 providers, 2,200 medical practices, and 1.2 million patients in its network, the company supports real-time feedback loops and shared accountability, shifting traditional financial models to incentivise value-based care.

CenterLight Healthcare’s interdisciplinary team spans doctors, nurses, therapists, and nutritionists, delivering culturally sensitive care across New York City, Westchester, Nassau, and Western Suffolk Counties.
